HIV Case Management
A primary focus of the Case Manager is to empower the client to connect with whatever resources are available and to enhance the quality of life in whatever ways are appropriate. As the spectrum of HIV changes, so does the role of the Case Manager.
Case Management can include help in accessing: Medications, Financial Services including IDPA, SSI, SSDI, ADAP; Support Groups, Legal Services, Substance Abuse Programs, Transportation Assistance, Medical/Dental Care.
Case Management also provides the client with ongoing information about living with HIV/AIDS, help in dealing with issues of loss, death and dying, emotional support for the client and loved ones.

Home Services
Since 1991, Open Door Clinic has contracted with the Office of Rehabilitation Services (ORS) to provide in home services to people with AIDS. This program was implemented to decrease hospital costs and to improve the quality of life for persons with AIDS. Services Provided
- assistance with activities of daily living such as housekeeping, meal preparation and laundry.
- electronic home response.
- home delivered meals - limited in some areas.
- respite care.
- assistive equipment.
Who Is Eligible
Any Illinois resident who has HIV or AIDS and is limited in caring for themselves in some way is eligible.
Who manages this program?
Open Door Clinic case managers are trained by ORS to assess and implement services. Minimum contact consists of one home visit per month and weekly phone contact to continually assess the client's needs as they change.
